Job Purpose:
The Direct Funding Community Facilitator provides support to adults with physical disabilities (called Self-Managers) who access an innovative self-managed attendant services program that is funded by the Ontario Ministry of Health.(www.dfontario.ca) Self-Managers on the Direct Funding Program, recruit, hire, and employ their own attendants to assist them with activities of daily living, including bathing, dressing, meal preparation and light housekeeping. The facilitator supports Self-Managers (SMs) through the initial inquiry, application, interview, management and wait list process throughout the Niagara and Brantford areas. The DF Facilitator will meet and provide support to SMs in-person in their homes as well as virtually via phone or video conferencing.

Qualifications

Qualifications/Knowledge/Experience Requirements:
 An understanding and acceptance of the Independent Living philosophy i.e. respecting a consumer’s right to take control of their own lives; examine options, make choices, take risks and make mistakes.
 Experience working with people with disabilities is an asset
 Diploma/degree in social services, or related human service/health professional degree
 Reliability, responsibility and accountability in fulfilling obligations to both the person(s) with disabilities and the organization
 Experience/some knowledge of payroll, provincial employment standards and hiring process, staff management and human rights an asset
Skills:
 Attention to detail and ability to complete tasks thoroughly
 Competent in working with persons with disabilities
 Reliable, responsible and dependable in fulfilling obligations
 Excellent communication skills, empathy, diplomacy
 Ability to organize, plan and prioritize work with minimal supervision
 Ability to follow direction and work co-operatively as a member of a team
 Competent computer skills in using Microsoft Office, Internet and email software, including Word, video conferencing, ability to learn custom database
 Ability to problem solve and make decisions accordingly
 Ability to make community connections with other agencies that provide services to persons with disabilities
