Responsibilities
• Provide care coordination and supports to clients referred internally and externally utilizing a variety of methods.
• Assist Clinical Team with expedient completion of a variety of assessments and forms (i.e. ODSP)
• Assist with the Client Concern process by ensuring there is follow-up and agreed upon actions are completed.
• Provide guidance and direction to other CHC counsellors on caseloads and overall counselling protocol.
• Assist clients to meet identified needs and actively lead and participate in their care plans. This includes referring clients to other social services or supporting clients in utilizing community resources including legal, medical care, financial assistance, housing, employment, transportation, day care, and other services.
Qualifications
What We’re Looking For:
• Certification and membership in good standing within a regulatory college (i.e. Social Work, Occupational Therapy, Nursing, etc.)
• Working knowledge of coordination of client care
• Experience in working in a community health setting would be an asset
• Certifications relevant to target clientele (addictions, acute and complex care) also an asset
• Experience working with people who are marginalized, socially isolated, living in poverty, and with complex physical and mental health needs, including newcomers and refugees, senior citizens, youth, individuals and families with low incomes, and people experiencing homelessness.
• Excellent clinical, communication, and problem-solving skills
• Proficiency in the use of EMR systems to follow established protocols and maintain accurate and timely documentation
• Commitment to continuous learning and professional growth.
• Ability to work flexible hours.
• Valid Ontario Driver’s License and access to a reliable vehicle.

• Police Reference Check.
Base Salary
$61,653 - $74,148 + HOOP pension, and other benefits listed below

Full-time, 37.5 hours/week
